AgriSafe Provider
Annual Dues - $150

  
AgriSafe Provider Membership is available to organizations that have a trained healthcare provider who has attended and passed the 40 credit hour agricultural occupational health training program and signed an Affiliation Agreement.

Organizational Membership
Annual Dues - $250
Organizational Membership is designed to support organizational entities that are concerned with agricultural health and safety.

Benefits include:

    + Member rate on AgriSafe services, products and educational resources

Individual Membership
Annual Dues - $50
Individual Membership is structured toward the professional who is concerned about agricultural health and safety but not able to operate an AgriSafe clinic.

Benefits include:

State Affiliate Membership

 
2011 Annual Dues - $1250
   

   State Affiliates provide leadership in the development of AgriSafe services in their state and are formed based on the same mission and vision of the AgriSafe Network.

   State Affiliation Benefits Include: 

     + Expeditites the process of disseminating information to rural health providers
     + Represents an innovative model of care that historically attracts funding
     + Empowers local AgriSafe providers to communicate state specific concerns to      colleagues with similar interests
     + Provides for a unified leadership role in future occupational health activities in each state
