AgriSafe Provider
Annual Fee - $150
AgriSafe Provider Membership is available to organizations that have a trained healthcare provider who has attended and passed the 40 credit hour agricultural occupational health training program. Clinic members receive benefits including:
+ Member only website with access to screening instruments and protocols
+ Access to internet-based distance learning sessions
+ Member rate on AgriSafe services, products and educational conferences
+ 25-40% discount on Personal Protective Equipment products
+ Various marketing benefits including radio and print advertisements
+ Affiliation on one or more committees
+ One vote on AgriSafe matters at annual meeting of members
+ Alive and Well Quarterly newsletter
+ Discounted rate on subscription to The Journal of Agromedicine
+ Member rate for the textbook - Agricultural Medicine: Occupational and Environmental Health for Health Professionals - Donham and Thelin, Blackwell Press, 2006
Clinic/Provider Affiliation Agreement
Organizational Membership
Annual Fees - $250
Organizational Membership is designed to support organizational entities that are concerned with agricultural health and safety matters but do not plan to operate an AgriSafe clinic. Organizational Membership entitles the organization to benefits of the Network including:
+ Access to internet-based distance learning sessions
+ Member rate on AgriSafe services, products and educational conferences
+ 25-40% discount on Personal Protective Equipment products for certified professionals
+ Various marketing benefits including radio and print advertisements
+ Affiliation on one or more committees
+ One vote on AgriSafe matters at annual meeting of members
+ Alive and Well Quarterly newsletter
+ Subscription to The Journal of Agromedicine

Individual Membership
Annual Fee - $50
Individual Membership is structured toward the professional who is concerned about agricultural health and safety but who is not able to operate an AgriSafe clinic. Benefits include:

+ Individual Member rate on educational conferences
+ Alive and Well Quarterly newsletter
+ Affiliation on one or more committees
Farmer and Farmworker Membership
Annual Fee - $50
Farmer or Farmworker Membership includes those who do not derive an income from work related to agricultural health and safety. Benefits include:
+ Access to internet-based distance learning sessions
+ Farmer Member rate on educational conferences
+ Alive and Well Quarterly newsletter
+ Affiliation on one or more committees
+ Discount on local clinic services and products
